I add all the number of citizens the PC would be able to save during "7 days..." and they would be saved only 2800 people so they can't obtain the last award, but I might be wrong (like radiohead).
I think that, during my party I'll give access to magic weapon or armor like suggest but to increase the "relation" between the PC and the Queen, I think that I'm going to end the adventure with a little speech of Ileosa to congratulate the PC for their help (even if we know that she hates them and wishes to kill them all, like Metallica).
Because my party doesn't like Ileosa, thinking there's something wrong with her and the fact that she congratulates them, they're going to think that the only reason she would do it, it's just to not appear like an evil person. So a lot of fun for me, of course :-)

If you add Brienna then that pushes them over the target number. There is also the possibility that the PCs will want to do things of their own initiative to help, and you can reward them a number of lives saved for those efforts. Really, the personal commendation from the queen is just for those who have gone above and beyond what they're paid to do.

I believe James said at one point that they didn't expect the players to get the best reward unless they went above and beyond what the adventure gave out.
Correct. We should have been more clear about that, but the idea was that only by going BEYOND what the adventure details (say, a PC devotes all his 3rd level spells to remove disease and goes around casting it, or a PC goes out of town or something to buy wands of remove disease, or if the GM wants to build more encounters for the PCs to go on, perhaps based on the "Other Plague Events" listed on page 44), they can earn enough body count to hit that highest tier.
The adventure itself pretty much assumes the PCs "win" by hitting the Local Heroes level of success, in any case.

Don't worry James, thanks for your answer, and if fact that is only because my party doesn't take initiative other than whose describe thaht I'm asking that!
The only "problem" is that when they became "local heroes" it is decribed that "Field Marshal Cresida personnally thanks ..." but in my party, Cressida and my party are very closed (according to what was written in the Pathfinder #7) so the fact that Cressida thanks them, it is a normally things during my party so they won't understand the real success they realized, that is why I want to make they thank by Ileosa because they don't like her (perhaps not only the PC personnally but "all people who worked to find a cure") and give them acces to magic weapon etc...What do you think about that?

You can have Creside Kroft gather all the guards and publicly thank the PC's in front of all of Korvosa's armed force (put aside the Gray Maidens). having the soldiers sulote in respect. that should be more aproprate then ant other option, IMHO.
