Not warehouse problems. We usually stuff envelopes with Pathfinders, modules, catalogs, etc. first. The various envelope configurations (PF-only, PF+GMM, PF+GMM+PS, etc.) go into separate piles, then we print out your the mailing labels and slap'em on an envelpoe from the appropriate pile. Takasi's label probably got put on the wrong envelope, or an envelope got put in the wrong pile. We go to great lengths to prevent this (we only use highly-trained warehouse monkeys), but things happen (they are, after all, monkeys).
This is how we ship the bulk of the subscription shipments, with a few circumstances causing a few shipments in each run to go through the standard (non-subscription) warehouse process (these ones will have an invoice). Therefore, unless your order shipped under different circumstances, you won't usually have an invoice in with your subscription shipments. For this Pathfinder run we don't have a new catalog, so there were none put in this time.
