From what your Perception rolls were showing Graxzos, no. However, Perception rolls can be aided by another (last turn no one other than you rolled over 10), so...
Round 2 (partial)
With Graxzos' help, Necress is able to find the spider hiding in the opposite corner on the ceiling. 15 ft above you.
After Necress points it out to everyone, Graxzos is able to shoot it. Will use the first readied action this round, then the second next round.
Feyandoria, Necress, Boderus, Creature (4 damage), Graxzos

Sorry Asmythe, I don't know how that happened. Corrected.

@GM I believe the perception roll was only for when we entered the room. If the spider attacks it gives its location away.
Boderus grabs his shortbow and fires at the creature.
Atk roll: 1d20 + 6 ⇒ (10) + 6 = 16
Dmg roll: 1d6 + 3 ⇒ (6) + 3 = 9
| GM Crash |
@Boderus: Alright, I can't tell from the scenario, but no harm no foul. I'll just remember that in the future.
Feyandoria's shot goes wide.
With a shot from Necress and Boderus, the spider falls from the ceiling and splats on the ground.
Combat over!
Looking around the room, the party finds a small cocoon holding the remains of an unfortunate dog that fell into the spider's clutches. It has a collar with lapis lazuli stones on it. The collar is worth 50 gp.
Venturing into the western room shows that it is mostly cluttered with rubble but it has a door that can be easily reached. Opening the door shows a stone stairwell that would run up and down...if not blocked by the stones covering most of the stairs that make traveling the stairs on this level unsafe.
So that leaves climbing the outside of the tower the only way to get to the next floor.

:) I asked someone who ran the scenario before because I was confused. He said that the roll is for the surprise round for the spider.
"Only one way left indeed."
He walks outside with the group.
"I have a rope but no grappling hook. We could climb but it's not one of my greater skills I have to admit."
| GM Crash |
With the rope, climbing will be a DC 10. With everyone taking ten (if no one dumped Str), everyone can make it to the second floor.
Boderus is able to hook the rope on a piece of the wall on the second floor and is followed up to the second floor by everyone (with everyone who wore armor taking it off to climb, then putting it back on when reaching the second floor). The party sees two doors, one to the northwest and one to the southwest. They also see that they can also reach the third floor with the rope.
Graxzos Glyphkeeper

Graxzos struggles with the climb, but does finally make it up, panting and puffing. "Too damned steep, ropes! I'm not a fan..." Glancing around, he snorts. "Looks like we've got options...let's see if there's aught we can tell."
Approaching the southern door, the dwarf examines it carefully visually, and then presses his ear to it.
Perception: 1d20 + 2 ⇒ (17) + 2 = 19
Boderus Flower

Boderus climbs up the rope with ease, after taking off his armor. When on the balcony Boderus sees Graxzos examining the door.
"Let's go into this floor first. I think the best treasure will be buried under the rubble below. Best start as low as possible then. Grax, is the door safe?"
Boderus will follow after Graxzos to go inside.
Graxzos Glyphkeeper
|
Graxzos, his loaded pistol drawn, advances cautiously into the room, looking about him as he goes; keeping an eye out for anything valuable. Or dangerous. Or valuable and dangerous.
"Follow me, mates!" he whispers, acting a lot more competent than he feels.
If he makes it to the other side without incident, he'll listen at that door as well.
EDIT: Forgot to make the roll.
Perception: 1d20 + 2 ⇒ (12) + 2 = 14
Graxzos Glyphkeeper
|
"Hrrrrm," Graxzos growls quietly. "Some sort o' lizard-like critter was all crawlin' around over here by this door...may be a lizard, or a kobold. Or a very small dragon..."
He looks up at the door and grins. "Guess there's only one way to find out!" The goggles come down.
Once the party has prepped in any way they desire, Graxzos is happy to kick down the door!
Boderus Flower

Boderus decides to ignore Asmythe's remark and readies his crossbow as the dwarf decides to kick down the door.
"Ready."
| GM Crash |
As Graxzos steps up to the door, he hears a twang and a javelin buries itself into the ground about an inch from his foot. Javelin Trap: 1d20 + 15 ⇒ (1) + 15 = 16 If it wasn't for the 1... He then continues his original motion and kicks open the door.
Racks for weapons line the walls of this chamber; above the racks hang various shields, pieces of armor, weapons, and standards on display. Most of the racks are empty, but a few weapons remain in place. A horrible stench fills the room as well, akin to the smell of rotten eggs mixed with the reek of an open sewer. Two humanoids with scaly, dull gray hide. Their frames resemble that of cave lizards, with long tails and crests on their heads and backs. Fort saves please. DC 11 or you are sickened.

As Graxzos goes to kick the door down, he pauses slightly when the javelin buries itself in the ground. "Huh. Guess we shoulda checked for that beforehand! Oh well. No harm, no foul!" And boom goes the door.
Fort Save DC 11: 1d20 + 3 ⇒ (18) + 3 = 21
Graxzos's nose wrinkles when the door opens to reveal the ugly menaces, but he doesn't particularly seem fazed. Seen a lotta corpses smell worse'n these...
Waiting to post action until closer to my turn in initiative, in case things change.

Necress Nyx

With nothing more than an icy stare, Necress wills the troglodyte on the back line to betray his ally.
DC 15 will save, casting murderous command. The spell has only verbal components, but because I'm deaf it has no verbal...I just will it to happen!
Graxzos Glyphkeeper
|
"Somethin' awful, no doubt," Graxzos comments as he strides forward. "Don' worry, though, 's about to go away. Ho! Beastie!"
Graxzos strides forward and fires right at the nearest creature's face!

Feyandoria becomes sickened.
Boderus becomes sickened but is still able to land a hit with his claw.
Graxzos isn't fazed by the smell and kills the harmed creature.
