That's correct, swarms automatically do damage to anything in their squares.

Mentally congratulating himself for restocking his supply of alchemist fire, Sebastian tries to toss one in the area not currently occupied by his companions. His aim is off, however, and only hits the swarm and Illidan with splash, "My apologies, good man!"

Targeted the red square of swarm that is two square west of Sebastian. He probably missed based on that roll, so it would hit the square south of the target square, the swarm and Illidan takes 1 point of fire damage.

I believe if the Sound Burst is a spell or spell-like effect, that would provoke attacks of opportunity from everyone adjacent to the Derro, unless he tried to do it defensively, but that would require a concentration check from him. If he fails to make those concentration checks, he loses the spell without it going off.

Turns out I read something wrong, you were supposed to do the opening of the tumblers *after* this encounter. My bad.

With a final audible click the tumblers lock into place. Suddenly, the fountain begins to rotate and its stale waters drain rapidly. The grinding of the gears echoes o. the stone walls, causing the ground to shake. As the fountain rotates, a spiral staircase appears where the fountain’s solid basin once sat. A slick staircase slopes down into Cassomir’s bowels, twisting its way down and around a deep cistern.

An enormous room lies ahead, filled with dozens of family crypts decorated with family names, heraldic crests, and a variety of death-themed statuary. The walls, floor, and ceiling are made of smooth, polished stone, and the crypts are made of thick, square blocks of granite. A handful of statues stand throughout the room, though they don’t seem to be placed with any plan—some face the wall, some lie on their sides, and others are damaged beyond repair. The northwest quarter of the room is dominated almost entirely by a single large crypt. The name “Ismacco” is displayed across the front of the crypt.

I'll try to get a map up during the day today, but if not it will certainly be up this evening.

Sorry to keep delaying, but I want to make sure one of the other PC's move into melee with it before Sebastian does. Otherwise he's going to be Otyugh food for sure. I'll post his actions once another PC moves in, or everyone else declares a melee action.
