Anyone have any thoughts on what to run after Strange Aeons to get the PC's up to Lvl 20?
The notes on pages 57 and 66 of Black Stars Beckon reference a few low-level scenarios, but I'm not sure scaling those up to double or triple their designed character level would make for the best end-game adventure.
Open to any 3rd party or old rule-set references.

I've actually been playing around with expanding the plot beyond the end to have them encounter a full fledged old god

Hastur as a rather glaring weakness.
He can be summoned by the play "King in yellow" and cannot refuse this summon.
Naturally, performing such a play in a place that can kill him is indeed something that may occur to certain entities, especially one such as Nocticula who specializes in killing demigods.
The King in Yellow has indeed been performed in Alyushinyrra, and Hastur died.
Only to respawn in Carcosa, where he was killed again. At which point Nocticula found out the hard way that Hastur is the sane and agreement capable of the whole great old one pantheon, and was keeping the lid of things that are much... worse then him. Luckily for much of the multiverse, she had a contingency for the eventuality of killing someone or something she shouldnt have killed, and was able to rewrite time, partially.
The vast majority of the multiverse only experiences a mild sense of Deja vue, and some unfortunate mimes in Alyushinyrra just get assassinated mid play, but Nocticula, and more importantly Hastur, still remember what happened.ü
And since Hastur is... reasonable, comparably, he quite reasonably strong disapproves of getting killed. There is an ongoing cold war between the Nocticulas demon Empire of the midnight Isles and initially just Hastur, but soon most of the great old ones.
Defeating Lowls put the party on the Lady in Shadows radar, and her agents have a business proposition for him. After all, there is, maybe, a way to permanently kill Xhamen Dor, and all of the multiverse agrees that only a more dead Xamen Dor is a good Xamen Dor, but it needs to be killed twice, at the same time, on different planes and one of the killers must be capable of trapping the soul of a demigod.
That would be Nocticulas "Job", but the other killers... must be mortal, and have willingly served Xhamen Dor, at least indirectly before, and there is a very short list of mortals who meet these criteria.
