Because the 4th-level version has +9 whereas the 6th-level version has a +12. So if the pattern is being consistent, then 5th-level version should have a +12, not a +6.
9 > 12 > 15, a proper linear escalation of 3s.
9 > 6 > 15 just looks like a typo to me.

There has been similar discussion before and apparently the math comes out OK with an steady rise in dmg even if the "+X" looks odd along the way.
So basically, what Grognard said. =D
Have any developers weighed in, in said discussions per chance?

Lvl 4: 1d12+9 ~ 15.5
Lvl 5: 2d12+6 ~ 19 (2d12 + 12 ~ 25 per Ravingdork)
Lvl 7: 2d12+15 ~ 28
Stegosaurus
Lvl 4: 2d8+9 ~ 18
Lvl 5: 4d8+6 ~ 24 (4d8+12 ~ 30 per Ravingdork)
Lvl 7: 4d8+15 ~ 33
So no, a +12 bonus would boost the lvl 5 damage to nearly the lvl 7 damage. +6 gives a somewhat smooth damage per spell level progression.

That ignores the doubling of dice though. Look at the other forms, if it is a mistake it is a consistent mistake.
I can guarantee you that the devs don't just look at the bonus in isolation outside of other damage bonuses the form gets -laughs-
