When using the Usurp Spell ability of the haunted spiritualist to drain the spell energy of the "transformation" spell:
How do material components work? Does Usurp spell need components and if so does the phantom need to have the components?
Is the character level refering to the phantoms level as it matches with the spiritualist level on the phantom progression table, the spiritualist's character level, the spiritualist's caster level or the phantom's hit die when determining the BAB granted by the spell?

How do material components work? Does Usurp spell need components and if so does the phantom need to have the components?
From my reading, material components work normally. As the Phantom (which the haunted entity still seems to be built around) can wield magic items (though not weapons) and wear items, it would seem that it would need access to any components a spell called for. Whether it takes it off the Spiritualist or has its own that it dons or grabs when it manifests is on an individual basis.
The reading doesn't seem to imply that spells become spell-like abilities or otherwise function any differently (though they cost the haunt twice the normal spell slots), so either have access to the components or choose spells that don't have components you can't use.

The Spritualist's level determines the Phantom's level.
For example, a 5th-level Spiritualist will have BAB +3, but will have a 4HD Phantom with a BAB +4.As for any specific abilities that use character level, class level, HD, etc. those will likely be based on the Spiritualist's level unless it's an ability being used by the phantom, which will use its own level (which will likely match its HD in most cases).
