I am converting the first book of this AP as a fun introductory adventure to 2e, and so far everything is going great. The doppelgängers are brutal against 1st level characters but I wanted to highlight how much more fun solo creatures could be and it is working great. One question I have is: should I give these ex-doctors the medicine skill? So far I have been able to have them hit and run pretty well and I can’t decide if they should be able to heal themselves or not

I think Oathsday is the only one to have really been a doctor, so perhaps she'd be the only one for sure to have the Medicine skill. I'm slowly converting this as well, and am just getting to this area of critters. Scaen got away but I'm not having her quick heal, so she's laying low. Picking Oathsday's abilities was fun as there is no Mesmerist class in 2e so I felt free to pick and choose what felt interesting. She would definitely be trained in Medicine.

Sorry Grankless, My players really wanted to go with something that was more silly and light hearted fun, so we switched from Strange Aeons to Extinction Curse after they defeated Oathday.
The gritty survival horror was largely being ignored and I wasn't really comfortable seeing a campaign set in a mental health institution turned into frivolous, mean spirited joking. The players are all old friends that are using PF2 to reconnect from around the country, so it just seemed easier to change the setting than to try to fight them about trying to take NPCs seriously when they were looking for an opportunity to goof around and not take anything seriously. The circus just made a better setting for that.

@Grankless I'm running this converted to 2e on roll20, I'm only converting a couple of sessions ahead of where the party is so I've only done up to the East Ward of Briarstone but if you want to bounce any ideas or see anything I've done just ask.
I should say it is my first game running 2e so I feel like i'm taking shortcuts (for example with monsters I'm just finding something similar of the right level and re-skinning it) not claiming it is the best way to convert but it's there if you want it.

Nothing wrong with doing that! It's working smarter, not harder.
And I'm mostly just in the brainstorming stage right now. I know I"m gonna make Cassilda be one of the players' witch patron, for instance. Personally I"m excited cuz I can use cool enemies that aren't just ghouls with how fun 2E enemies are.
Maybe there should be a different thread for this? Who knows.

It sounds like you are on the right path
I think one of the mistakes I made early on was trying to keep too close to the 1e information (attacks, DC's, etc.) rather than using the 2e damage levels and conditions but still describing them using the terms given in the adventure path.
