My characters are approaching Thistletop (and one just fell from the trapped bridge...) so I made Nualia using 2e rules.
Thoughts?

Channel Smite Expend a harm spell. You siphon the destructive energies of negative energy through a melee attack and into your foe. Make a melee Strike and add the spell’s damage to the Strike’s damage. This is negative damage. The spell is expended with no effect if your Strike fails or hits a creature that isn’t damaged by that energy type (such as if you hit a non-undead creature with a heal spell).
Harming Hands When casting harm, roll d10's instead of d8's.
Tactics: Nualia expends her Harm spells in descending order to Channel Smite with a Two Handed attack on anyone showing religious preference other than Lamashtu or the strongest looking character. She attacks with sword (one handed) and claw if all of her Harm spells are gone.
Morale: If her health drops below 40, she uses Vampiric Touch and then Heal on herself. I reduced to 20 hp or lower, she cast Sanctuary, Darkness, and Fear in an attempt to escape.
Description: With the exception of her demonic hand and her scarred belly, the rest of her body is incongruously beautiful. Yet in her madness, Nualia has come to view her silver hair, violet eyes, and shapely figure as a curse, a scar visited upon her by her angelic heritage. She wear the mark of her devotion to Lamashtu proudly, keeping her midriff bare to expose the ugly scars and wounds across her belly.

Have you seen the monster creation rules? These give a benchmark for what threats of each level should look like
For a boss fight her attack, damage and saves seem too low
Indeed attack and damage is really bad for ostensibly a melee threat
Realistically you shouldn’t use the standard PC creation rules for enemies . Have a look at other level 5 threats as a comparison

I actually want to take a shot at this but am at Work. I will try and get on tonight and build something with the monster rules. If she were going to be a player this is an obvious conversion but in the name of optimization for 2e I would change the bastard sword to a Falchion Lamashtu's favored weapons since 2E clerics prof with weapons only improves for their deity's weapons.

Or she could just have an attack bonus appropriate for he level with no attention paid to class proficiency...

Oh for sure. I kind of meant those as separate statements of Class rule vs npc rules. I would defiantly try and carry over more of her abilities from 1E Her fury of the Abyss and ferocious strikes kind of work like 2e rage and she does have a heavy anger component so giving her an ability close to rage could be nice. Also her claw would agile so if she is ever out of spells/channels she could use that as a better chance to hit 2nd attack.

Your ac is reduced by -2 however you recieve +2 to hit and damage and deal +1d6 evil damage with sucessful attacks.
Also of note she should have her Yeth Hound with her. It isn't in the 2E Bestiary but the hell hound is so just give it flight and trade its breath weapon for Bay that gives the deafened condition for a few rounds with a DC 17 fort save.
