We know they mostly live on the Prime Material Plane, just in outer space somewhere. I always thought that they live in the quasars, as Earth apparently exists in the same universe, and the quasars are the most distant (and thus oldest) objects we know of (even the closest one is about half a billion light years away.)
That's what the Dark Tapestry is- a big dark ominous part of space between the stars where the Outer Gods live.

Different places. Azathoth, for example, lives in the center of the universe, while Yog-Sothoth lives in the Dimension of Time, and Nyarlathotep lives in orbit around Azathoth but spends most of his time on various worlds wreaking all manner of mayhems.
