Page 417 of the CRB reads the following:

"Frequency: This is how often the periodic saving throw must be attempted after the affliction has been contracted. If the affliction lists an amount of time after its frequency—such as 1/minute for 6 minutes—that means its effects last for only that amount of time, regardless of whether the affected creature ever succeeds at a saving throw. Such an affliction cannot be cured via successful saving throws; after its duration ends, the victim remains at her current step on its progression track until she receives the benefit of remove affliction or a similar effect."

I am not sure I understand. In the example given, would the affliction only last a maximum of 6 minutes or would the affliction and its effects last until a remove affliction or similar effect is received by the character?

Moving down the affliction path is determined by the saving throw. Let's take a look at a Dex track:

Healthy—Sluggish—Stiffened—Staggered—Immobile—Dead

Let's say you fail the first save, so you're now sluggish, make the next 2, fail the 3 (which makes you Stiffened), and make the last one. As the 6 minutes is done, you no longer have to make any saves, but you're still stiffened. That affliction would need to be healed by your friendly neighborhood mystic.

If you failed all 6 you'd be dead (as that's the bottom of the 6 steps).

For poisons and drugs, you continue to make saves until either:

1. The duration expires
2. You make the number of consecutive saving throws required
Once one of those two happens, you stay at your current stage on the affliction chart. After that you need rest. Each day of bed rest or two days of normally activity moves you back one step on the affliction chart.

Diseases do not have a maximum duration and you continue to make saves. Each time you meet the "cure" condition (such as saving vs. bubonic plague 2 days in a row) you move back one step on the affliction chart. When you return to the "Healthy" stage the disease is fully removed.

Curses are usually "one-and-done" saves. They have a unique cure condition, and once you meet that condition, the curse is completely removed with no lasting effects.

And of course magic can effect them all.

The "that means its effects last for only that amount of time" part of the rules had me confused. I didn't realize that "effects" here referred only to the those which cause the afflicted character to make saving throws.

I’m running an online Dead Suns AP and I just figured out how I was doing afflictions/diseases incorrectly w/r/t void death. It took me a few weeks to realise it though. There’s nothing in the AP that says “warning: diseases work differently in Starfinder”.

D’oh!

I guess my reading comprehension score for the Core Rulebook sucks.
