I'm looking for the conditions that modify stealth vs perception. For example: Darkness, Distance (perception penalties by range), prone vs standing, etc.
Disclaimer: I'm not taking into account magic spells / items or class abilities like a Shadow Dancer, just looking for mechanics which would effect Stealth vs Perception.
Most of the rules for this sorta thing seem to be scattered all over the place, I am also wondering if any one has compiled a resource where they are pulled together in one place? If not, this might be a good time to make such a list <grin>.

Before anyone asks, yes the rules for stealth and perception have already been reviewed and are assumed as a baseline for this discussion.
