Some may have seen the channel on youtube, CinemaSins, where they point out inconsistencies, errors or mishaps in the movies, accompanied with a pretty counter.
Soooo....there's of course silly things that have happened at your tables, as a player or a GM, that had unfortunate or unneeded outcomes.
Outcomes that, if attention had been paid, or common sense would have been applied, would not have ended that way.
Could even be written tactics not making sense..... :)

Here's the 1st:
Party finds statues.
#1 [Insert sword for power] > +1 to dmg.
#2 [Insert head for knowledge] > +1 to int of the pc.
#3 [Insert sword for power] > sword breaks, to PC dismay.
#4 [Insert head for knowledge] > Decapitated PC...
> You'd figure the sword alone was warning enough.

GM: Two figures come shuffling towards you, moaning.
They don't act like zombies though. (Hints at inspection)
Lawful PC ignores the info and smacks down 2 innocent traumatized commoners.
