If a cursed item has the drawback "Character is polymorphed into a specific creature (5% chance [01–05 on d%] each day)," how do you reverse the polymorph?


I am only going by what you've written here and I don't have any specific cursed item details, but the general answer would be:
if the item doesn't specifically say how that effect can be ended, then any effect that can normally reverse or remove polymorph effects should work. Even a dispel magic.

Unless the item says something like, 'only a limited wish or greater can reverse this' or, 'only a remove curse cast by a 12th-level or higher caster' then anything that removes polymorph should do it, even another polymorph effect unless it's like baleful polymorph (which it probably is) in which case other polymorph effects won't work while you're under its effect.

Otherwise, unless that effect has a duration, once it happens (5% chance per day) it's happened. Depending on the item it may or may not merge into your new form and if it has any continuous ability (other than shield or armor) they may still even work.


As a general rule of thumb i think it would be a Remove Curse spell that beats the Cursed Item's caster level in the Caster Level Check when cast. Other spells like Break Enchantment, Limited Wish and above can also do it a lot of the time.
