Mass Combat--Army Sheet...?

I'm looking over the Mass Combat rules and the Army Sheet. The Sheet shows both an Initiative Modifier and a Max Armies box, but neither of those values is mentioned in Ultimate Campaign, as far as I can see. Can someone clue me in?
