Aquatic campaigns / underwater

So for the people who ran aquatic or underwater campaigns, what stuffs didn't you expect? Did you actually bother fighting with the 3D combat?
Some things that you were disappointed with? I'm just curious, it's the type of campaign that seems to never get much spotlight.
