So, most AP's take the characters to 15th-18th level. What do you run after the AP is done, if the players want to keep going?

I play with kids so i've never made it to an end:-)
they generally lose interest in the middle:-)

It depends on the AP. They all have recommendations on where to move after they finish.

Dig out a copy of Dungeon #100 and throw a full blown Incursion storyline at your PCs with Denizens of Leng in the place of Githyanki. The Lich Queen's Beloved final chapter of the Incursion is designed for level 18 characters.
