It may not be fixable if the record doesn't exist. Email the VC for Indianapolis and he may be able to point you to someone to contact about getting your table entered. Provide as much info i.e. table number and slot number if you can.
I've always operated on the honor system in the tables I run. If you were to tell me this story, I'd be inclined to believe you, let you have your Chronicle, and get on with the game.

The paper is the proof. Electronic records aren't always right. I have a Gen Con event posted to the wrong character (the character was already dead...) I once had a game from a convention in a state I haven't been too since before Pathfinder existed recorded with my number.
What matters is you have the paper copy and if you really want too, you can scan it to save a record of it.

The reporting for the Quests at Gen Con were sketchy, as the GM's from the first slot (Me being one of them) didn't even know what to do with the chronicles at first and was not told to report the players. We was told about half way through the slot that we was giving away the cards and putting checkmarks on them. Those were being turned in for the chronicle at some desk somewhere.
I am gonna have my games (I still have my "card") put on a chronicle once I get a chance to play a couple of the quests, adding in the GM credit to the rest.
Not sure how the reporting is now.
