On the card game Chronicles sheets, I assume the goal is to reuse the chronicles sheets.
Is the intent to have one chronicles sheet with all you successes and then have all your failures on supplemental sheets?
So if I fail the first time at SS 1.1 Ghosts of the Deep, that would go on my failure sheet and the second time I play it and win that would go on a success sheet?
Then if I fail twice at the SS 1.2 Lone Shark before winning, the win would go on the success sheet, the first failure would go on sheet failure 1 and the second would go on failure 2?
Is that the general plan?
It was not clear at all in the FAQ.

Think of the Chronicle sheets for the Card Guild as log sheets for your characters. You can write the scenario name in the grey area at the top of each entry. Don't use separate sheets for success and failure, just track your progress as you go.
The first time you play S1.1, in your example, you'd record "Success? No" in the first entry of your A1 Chronicle sheet and list your deck upgrade if you took one. If you played it again immediately and succeeded the second time, you'd use the next entry on the A1 Chronicle sheet, write the scenario name in again, and record "Success? Yes," what you got as a scenario reward, and a deck upgrade if you took one.

Where do you get the chronicle sheet from or do you just use the sample sheet from the guide?
Chronicle sheets come with the scenarios. The person running your session should be providing it to you. (As well as signing it and reporting it.)
I still have questions about people that don't have a PFS number yet. How do you report their session if they don't?

You need to get them a number.
Either they can create a Paizo account on the spot and generate a number, or the coordinator can provide up to 10 "reserved" PFS numbers to hand out, with codes and a URL for that person to link it to their account.
You can get them under My Account -> Make Changes (under Pathfinder Society) -> GM/Event Coordinator tab

So, I went to find the link to reserve numbers, and it's nowhere to be found on that page. Am I missing something?
Need to make sure you've created an event first. Then it's at the bottom of the page. That's how I found it. Created my first event with all the details then the link appeared for me.

I just made a new event for another open game night I expect is happening on Thursday and it lists 0-0 through 0-1f, are you only seeing 0-0?
The demo is not a reportable event, so I don't believe that it will ever appear on the event creation screen. If you plan on doing any of the other scenarios in the future through using the same event but playing in another session (a second game that same night, the same scenario or a different one another night, etc.), you should just add all 7 of the currently listed scenarios to allow for easier future reporting. This will also give you the ability to print the cards as referenced above without having to go back in and delete the event.
It isn't spelled out clearly, but you can always add more sessions to a previous event and group them together, you do not need to create a new event for every game night that you have, especially if you make one event code for each location you play at. That is how I group them here, we do the same thing for PFS.
