Note that my mythic progression for the PCs is far slower than in the book, but they gain something every time they defeat a mythic foe.

What I'd do for Mythic is the following:
Erylium and the Runewell bestows the initial Mythic Ascension.
The Barghast under Thistletop is the first Mythic Trial (Tier 2).
Xansesha on the Clock Tower is the second Mythic Trial.
Bringing a body part of the undead Ranger to the ghost Nymph is the third Mythic trial (Tier 3).
The Big M and the first "encounter" with Karzoug is the fourth Mythic Trial
Finishing the Runeforge is the fifth Mythic Trial (Tier 4) - only allow one weapon to become enhanced unless players are spending their Tier for a Legendary Weapon at this point.
I'd say that four Tiers should suffice. If you want to push for a fifth, then you could add things like the Wendigo, the Demon in Xin-Shalast, and the guardian of the entrance to Karzoug's Runewell to get to Tier 5.
As for Karzoug, I'd give him a pair of Mythic Blue Dragons and give him several Tiers of Mythic above the players as well - you could have the "weak spots" that the Dominant Weapon can detect as a means of draining Mythic Tiers from Karzoug, rather than draining levels.

Okay, so the players haven't been Ascended yet?
Then I've a question for you. Is Nualia still alive? If so, have the town alert them that someone heard something in the Catacombs. Have them investigate, discover Nualia with a small band of mercenaries, and she's reactivating the well. Time it so that she activates it and it explodes with power that Ascends everyone just before they kill her or after they've killed all of the mooks.
(This is what I did to Ascend my crew. This was actually after they fought Xanesha, mind you.)
Alternatively, you could have them investigate the Misgivings and when they reach the final room, find Aldern is gone but he left a journal explaining how his mistress has commanded him to break the well (which should hopefully be deactivated by now). They arrive at the Catacombs, fight him and a couple ghasts... and his death causes the well to surge, giving them all an Ascension. The well then deactivates again. ;)
I like the use of the Runewell for the Mythic Ascension because thematically it works. The well is a minor artifact of ancient power.

I think the cleansing of the house in conjunction with killing Aldern (i.e. removing Vorel's influence as well) could be made into an encounter qualifying for "mythic"--might take some effort from your side.

I'd hold off. As I said, the Runewell is a much better vector for Mythicness and makes sense for the story as a whole. So if you want to bring Mythic into the game, use the Runewell. (It even explains Karzoug, by the way - he's not just trying to re-enter the world, his Runewell is helping him achieve Mythichood and perhaps even Godhood.)

Hehe, good question, bigrig107. A question I have no answer for.
And I agree with Tangent101 that the runewell makes for the best--most atmospheric--place for ascension. As you can read, I also opted for the Erylium encounter (using the runewell) for their ascension.
If my luck holds, the PCs will not come up with any measures how to block access to the well in the long run, thus I have at least a second chance for such an event.
