Slam attacks: How are they calculated

For example, small earth elemental has strength 16 and BAB +2.

It's slam attack is: Melee slam +6 (1d6+4)

Wouldn't to hit bonus be STR+BAB (+5) and damage 1d6+3? Are slams given 1 1/2 strength bonus on all creatures like a two handed weapon attack?

Trying to calc stats for some summon monsters with augmented summoning for Gen Con and want to be sure I get it right.

The small earth elemental gains a +1 to hit because auf its size. Its damage is more that STR bonus due to the following rule:
