One natural attack per limb and you can't use a sword in a hand making a claw attack.
The answers to your questions therefore depend on how many limbs the character has.
If he has two claws and a bite from DD and two claws from another class and a sword.. then, he'd need 5 hands to make all the attacks- plus a head with a jaw for the bite. (6 hands if the sword is a 2 hander.)

The Dragon Disciple has access to his/her Bite and two Claw attacks. Assuming the base attack bonus of this character is 6/1 or higher s/he can make multiple attack with manufactured weapons or unarmed strikes.
Selgard is correct. When attacking with both claws you cannot use that hand to attack with a manufactured weapon. Monks can always make unarmed attacked even when their hands are full, I'm not sure if this is the case with Improved Unarmed strike.
When making attacks with both Natural Weapons and Manufactured ones all Natural Weapons are treated as 'Secondary' and therefor have a penalty of -5 to all attack rolls and use your full base attack bonus.

So is the five attacks applicable only if the DD has 4 arms?

It depends on where you put the claws. Nothing says that the claws have to be on your hands. Put them on your feet and you are in business.
One last question - sorry.
So lets say the 4th level DD has claws on his feet and has 2 attacks as part of his character. Are the natural attacks allowed in addition to his 2 normal attacks or does he have to choose which 2 things he will use to attack with?
How are the 5 attacks calcualted in the above example?

You should be able to make five attack as a Full Attack action.
This is assuming the claws are on the feet. However, never make that assumption. If your claws are on your feet, I suggest showing written proof that it is possible.

I think James made a post once mentioning that claws go on a bipeds hands and talons would go on the feet
RAI is that they go on the hands but if your GM is fine with it it's not a game killer
This is correct, James clarified that RAI, claws = hands and claws on your feet would be called talons, which I think only birds actually get. In my opinion, putting claws on your feet is a bit cheesy since it's almost always done for the purpose of getting more attacks than you should otherwise be able to get at a given level. But if your DM is ok with that, so be it.

The fact that there is more than one type of natural attack that is assigned to a limb:
Gore (Head)
Bite (Head)
Moreover, there are different types of natural attacks that a limb can perform (slam/claw) which the only real difference is the damage die and type.
A slam is a blunt attack using whichever limb assigned (arm) and a claw is a slashing/blunt attack using the arm as well.
