in a grid such as...

...if I am on square '8' and I grapple my foe who is on '5', do we each occupy our own square or do we now share a square?
If we do share, which one?
If we do not share and I successfully maintain the grapple and move to square '6', where is my foe? Square '3'?

You no longer share squares as part of a grapple by default that's a 3.x holdover idea I imagine. So you would each stay in squares 8, and 5 initially.
After you maintain the grapple, and choose the move option, you can reposition the grappled target into any adjacent square (you pick), if you pick a hazardous square the target gets a free escape attempt at a +4 bonus.
