I'm not seeing anything that says you can resist a madness bomb or be immune to it. It deals its wisdom damage if you get hit by it.
It is an Su. All bombs (unless they say otherwise) are Su from the rules on alchemists talking about bombs in general "Bomb (Su): In addition to magical extracts, alchemists are..."
Normally bombs allow for a reflex save from the splash damage of the bomb "Those caught in the splash damage can attempt a Reflex save for half damage. The DC of this save is equal to 10 + 1/2 the alchemist's level + the alchemist's Intelligence modifier."
But those taking a direct hit get no save, and the madness bomb wisdom damage only effects the direct target.
As for flanking, the only requirement is threatening attackers be on opposite sides. There is no front/back or left/right designation.

This is considered ability score damage. It is described in the glossary of the Core Rulebook
How do you know if you are immune to it?
Generally you would need something to make you immune to Ability Damage to Wisdom, mental ability scores or ability scores in general. As a group the Constructs Creature type and the Behemoth and Inevitable Subtypes are immune to ability damage.
