Stat block for Iq’lothatuaa, Insane Broodchief

Currently running the Carrion Crown campaign and a bit confused on the stat block for this NPC. The gear carried by this creature is masterwork heavy steel shield and Medium +1 defending greataxe. Other information about this creature is Male Giant Skum Barbarian (savage barbarian) 6, CE Large Monstrous Humanoid (aquatic). So, the stat block shows the AC as 19, touch 10, flat-footed 17 (+3 Dex, +7 natural, -2 rage, +2 shield, -1 size). The melee attacks are +1 defending greataxe +14/+9 (1d12+13/x3), bite +13 (1d8+4), claw +13 (1d6+4) OR bite +15 (1d8+8), 2 claws +15 (1d6+8). The feats are Cleave, Improved Bull Rush, Intimidating Prowess, Multiattack[B], Power Attack.

So, my question to the Paizo world, is how would this creature be allowed to wield both a heavy shield and a greataxe, and still get a claw attack?

Oversight, I would guess. If he doesn't have a hand free, he can't make that claw attack.

Unless he has 3 or more arms?

I think the greataxe is medium size, not large size (large sized greataxes do 3d6 damage). Thus a large creature could wield it in one hand with a -2 penalty.

The Greataxe is indeed medium size. His stat block for that portion is correct. I agree on the oddity of the claw. I suspect they listed it for if you drop his shield/greataxe. Or something. Probably because his stats list him in rage, so they wanted to cover all their bases.
