Wait, you're asking whether stacking those abilities allows you to take two AoO for one provoking action? I don't think so.
At 11th level, kensai can make a number of attacks of opportunity in a round equal to his Intelligence modifier (minimum 1). This effect stacks with the Combat Reflexes feat.
It says it stacks, presumably in terms of number of AoO gained, but since it doesn't say anything about taking two AoO for the same action I'd assume that it works like Combat Reflexes in that:
This feat does not let you make more than one attack for a given opportunity, but if the same opponent provokes two attacks of opportunity from you, you could make two separate attacks of opportunity (since each one represents a different opportunity).
Sneak Attack damage absolutely applies on every attack, assuming the conditions for sneak attack are met.

Yes, but you can also take those two attacks for two provoking actions (casting a spell, making a ranged attack) if you have just Superior Reflexes or just Combat Reflexes.
And a Kensai standing next to a guy casting Bull's Strength doesn't get to attack him twice for casting one spell (provoking once) just because he happens to have both Superior Reflexes and Combat Reflexes.
Said Kensai will probably have a ton of AoO in a round, though.
