If temporarily unable to wear his chains, he may carry rocks or some other heavy burden (including medium or heavy armor) to simulate this suffering. Since shackles aren't on the list of items a Vow of poverty monk can take he must select them as the "other item" or can he include them in his "simple set of clothing"?
If your GM agrees, one workaround might be a willing party member who buys the chains and lets you wear (read: "carry") them out of respect for your vow.
Hm, another workaround could be that he is an escaped slave, and vowed never to remove his chains as part of bringing attention to the plight of slaves. He doesn't own the chains or claim ownership, he just wasn't willing to let the rightful owners use them to enslave someone else.
Just an idea!
I'd allow both without any adjustment. I don't feel the shackles go against the spirit of the VoP. I don't see anything game breaking about allowing them as a 7th item.
