I like to have them come without gear, but that harkens back to 1st ed when such followers would be gained at high levels.

It makes a kind of sense that an NPC in a bad position like that might want to seek the help of a higher level character..

The PFCR says:

"The cohort should be equipped with gear appropriatefor its level..."

I read this to mean that the cohort rocks up ready to roll with that equipment, because you can always outfit it with gear that's inappropriate.

Having said that, my players rarely take the feat, and when they do it's an NPC that has already been present in the story already.

I've always played it that they come with NPC gear for their level when they first are gained, then the PC is responsible for keeping their equipment up.

That does mean over time either the PC or the cohort will likely be underequipped; I'm ok with that as Leadership can be a very powerful feat.

Looking in the PRD the official answer is similar:

Cohort Level: You can attract a cohort of up to this level. Regardless of your Leadership score, you can only recruit a cohort who is two or more levels lower than yourself. The cohort should be equipped with gear appropriate for its level (see Creating NPCs).


In addition to my post above, I also consider Leadership to be a feat that should always be interpreted however the GM wants it to work. So long as that intertpretation is made clear to the players before they take it, all is good.
