Into a Melee or Fighting Defensively?

A wizard is fighting with an orc and the orc is close enought to hit the wizard with his sword. If the wizard cast a ranged touch spells to atack the orc is this considered a ranged into a Melee or Fighting Defensively?

I assume you mean casting defensively and attacking in melee with a ranged attack ?

You provoke an attack of oppurtunity for casting and making ranged attacks both.

Failing to cast defensively will ruin your spell, as does failing your concentration check when hit for damage with the attack of oppurtunity if you choose not cast defensively.

You provoke an attack of oppurtunity for making a ranged attack as normal, though most creatures can only make one attack of oppurtunity, this has no chance of disrupting your spell by damage but it is resolved before you make your attack, so if you are knocked below 0 hitpoints the attack will not happen.
