LOL, yes I am thinking ahead...
As we have all experienced in MMOs, there will be an percentage of powergamer/griefer playerbase no matter what GW does.
Not only that but even the most diehard IC roleplayers just wanna go kill something just to kill something every once in awhile.
So I introduce to you PFO's first expansion area: The Worldwound
Here the roleplayer can go with his alt character and join the war against the spread of demons to get his "fix".
Here the powergamer can go when the sandbox doesn't appeal to him.
Here the griefer can go when he gets put in "timeout" by GW. Sort of a weregild to work off if you will.
A virtual battlefield map will keep track of the spread or decline of the demon advance based on the players actions.

I'm not sure how much of your post is tongue-in-cheek, but I've often wondered how the map for PFO will expand...
Will the original starting area forever be the north-west limit of the map? Will it be the center?
I wonder how much the answer would impact how players choose where to try to build their Settlements/Kingdoms...

Ryan did say that they were not going to allow player settlements and kingdoms at launch, and not for awhile after. Hopefully we/they get a good picture of what the expansion route for the map will be before they come available. Otherwise, this is a very good point. Major suckage would ensure if you built your mighty kingdom on the edge of the map that never expanded, and your rival kept getting expanded land next to theirs.

Expansions need not necessarily equate to one entire map. Generally, yes, that would be what sandbox would mean however let's take into account the size of Golarion. It might make more sense that we can get access to different areas of the world, linked through fast travel.
For example, the first 'expansion' might be to the Isle of Kortos and the city of Absalom. You could fast travel to the Inner Sea's edge and take a boat over. After all, how long would it take to ride even the fastest horse from Crusader's Road to the edge of the Inner Sea to take a boat to Absalom?
While it might break immersion, I'd rather have the ability to 'quickly' navigate the world to see the things I want to see and do over spending one or more nights traversing the world.

Make Worldwound into a dedicated PvP (or PvE) area. With possibility of playing demon invaders. Let the current balance of power in the world wound influences events in the normal game area, for example:
- demons gaining dominion would increase number of certain types of monster spawns that could be associated with them (which would also mean that source of demonic materials are more common).
- demons gaining dominion would increase efficiency of dark magic
- crusaders gaining dominion would increase numbers of events related to crusade (for example presence of NPCs that buy supplies and send them to help the crusade) but it could also increase numebrs of different types of monster spawns than demons.
- crusader dominance could also decrease efficiency of dark magic or increase efficiency of holy magic.

I both agree, and heavily disagree with this statement. I agree that fast traveling does make it easier and allow players more time to experience what they want, however the thought of "I would like to see X, so I am going to spend a few days traveling to X" makes me cringe with excitement. (Yes, I said cringe with excitement.)

For me, the instant-travel question should come down to whether you're in a PvE or a PvP environment. If it's pure PvE, then there's no reason not to just open the map and click anywhere on it to be there. But if you're in a PvP environment, especially one where it's important to move resources around the map and to be subject to ambush while doing so, then it becomes very hard to justify ubiquitous instant travel.
I think the current plans for PFO are appropriate with respect to Fast Travel.

you don't have to break immersion to institute "fast travel"
have a magic portal that teleports you from a main hub to another main hub (not real sure if they are considered common in Golarion but PFO is stretching some PFRPG rules so might as well put a magic portal in that can magick me from Thornkeep to Absalom and back)
could be a gold sink too (charge 100 gp to use) ... or even a player kingdom with a mage 20 could build their own portal inside their fort
