I would say that since Major Image has a duration of concentration + 3 that the image would last for 3 rounds after ceasing to concentrate. So, you would have have to concentrate for at least 1 round in order to cast the spell but after that, you get 3 rounds of the image for free before the effects cease.

No. The image remains, but doesn't move if not directed. This would allow Mr. Bad Guy another save to disbelieve it.

If he thinks he's fighting a monster, but it suddenly stands still as a statue and doesn't react to his attacks, he might figure out something's wrong with this picture. All the same, he might think he's stunned it, or there's some other magic making it act that way, and keep attacking.

That's how I call it, anyway. Illusions are a lot better delineated than they were back when I was a young pup, but it still requires some GM calls. Someone else might see it differently.
