It sounds like he's probably under the effects of an enlarge person or similar spell that is increasing the damage die of his weapon. Being that this is in the 3.5 forum, I'm guessing this is NOT using Pathfinder rules, which is why the power attack is so high.
Smite evil = 4 damage (x3 crit = 12)
Power Attack 4, 2-handed = 8 (x3 crit = 24)
STR mod, 2-handed = 6 (x3 crit = 18)
Bless Weapon doesn't add any damage.
12 + 24 + 18 = 54 + 6d8
Getting 39 damage out of 6d8 is on the high side but not impossible, so yeah, the math is right. What exactly is your problem here? Yes, weapons with a x3 crit multiplier are disgusting, especially using the old (horribly broken) 3.5 Power Attack rules. He's only threatening a critical hit on natural 20s, though, so it's not like this kind of thing is happening all the time.

Human Paladin lvl 4:
Strengh 18
Charisma 15+Rol hit die and result 20, confirm with 19.
Weapon: 2h bludgeon weapon (2d8, crit x3)
Smite evil + bless weapon + power atack of 4 + strengh modifier = 93HELP. :(
I assume you're the DM, else the damage seems adequate. ;)
Smite evil +2 to hit, +4 damage
Bless weapon: does nothing regarding the damage other than in specific circumstances.
Power attack: -4 to hit, +8 damage
Strength: +4 to hit, +6 damage
So he is +6 to hit (+4 BAB, +2 smite, +4 str, -4 feat), doing 2d8+18/x3 damage (+4 smite, +8 feat, +6 Strength)
As a crit, it does 6d8+54. To get to 93 points, he did 39 points on the dice; average would be damage of 84 on that hit. It's a 1/day attack that also consumes a 1/day spell.
Assuming some normal other abilities (Str 18, Dex 12, Con 14, Int 8, Wis 12, Cha 15), this is about a 35 point build; this is a point system that implies a very dangerous campaign with high threat levels...maybe something with EL6 encounters as a regular diet and BBG encounters at EL8 for four characters.
So, what do you need help with? It's an atypical result. It is one that he was probably really excited about. It's one that probably killed your BBG or something, but is unlikely to happen that way regularly.

Assuming the "cheater" is a player, why not use a Cyclops as a monster?
Melee Attack = Greataxe +11/+6 (3d6+7/×3)
Use Flash of Insight on the charge, Auto-Crit-Threat, rolling +13 to confirm that.
That's 9d6+21 damage right there, or 48 on average.
Throw in Power Attack -3/+9, and you're confirming at +10 to hit & rolling 9d6+48, or 65 damage on average.
With a Con of 14 (assumptions, I know), Toughness, and Favored Class for HP, your Paladin problem just ate 65 out of his 40 (average) HP in one blow. All from a CR 5 solo monster.
Alternatively, talk to said player, and determine just how much playing with 3.5ed material is worth. Switching to PF and dropping a lot of the crappy end-of-life material from 3.5 will alleviate some of your problems.

Other than the unknown weapon, the abilities in question are all PHB (paladin smite, power attack). It is a low probability outcome. It doesn't require system-hate to address. The irony of suggesting the problem is broken material from 3.5 and then suggesting that the solution is a creature from the same sort of nova result problems from PF is palpable. :)

Yup. Everything here is pretty bog-standard 3.5 as Howie says. The weapon, as far as I can deduce, is probably a maul (essentially a bastard sword that deals bludgeoning and has x3 instead of 19-20/x2) benefiting from enlarge person or similar magic. I believe the maul was in one of the equipment books for 3.5, and I know it was in the Forgotten Realms campaign setting hardcover.
Either that or it's some kind of homebrewed monstrosity. Regardless, you should be thankful he wasn't using a scythe.
