I've noticed there is no swarm template in core books. After doing a search it had been pointed out that a third party book has it, but I don't have access to that, so I ask for assistance making a monster into a swarm.

You may not need anything other than the Beastiary (PG 312-313) to turn the Cacodaemon into a swarm with the Swarm Sub-Type.
The Advanced Beastiary's "Creature Swarm" template is fairly similar to what is shown in the Pathfinder Beastiary. The peice that is a bit vague in both sources is hit-points. For that you pretty much have to choose based upon the CR you are shooting for. If I were taking a cloud of the little buggers out for a spin I'd probably hit at least the 11-15HD range for 3d6 Swarm Base Damage (couple that with their Cacodaemonia and you have a pretty cool swarm). Obviously there is nothing stopping you from going lower or higher on the scale depending on the challenge you want, but I try not to create a swarm that would be overall less impressive than just several of the individual creatures acting independantly. As always YMMV.
