In my opinion, it looks like a Horned Devil.

It is indeed an illustration of a horned devil. By the time we got to actually developing that adventure, the art for the cover was done but we'd reached a point where it was obvious that a horned devil would be ridiculously overpowered to throw into the adventure, especially since he's not the boss. So instead it's basically just a generic "heroes fight a devil" picture that should evoke part of the excitement of the adventure, but no longer illustrates a specific scene taken directly out of the adventure.
