Total Defense gives a +4 Dodge Bonus to AC, which applies to CMD.
Fighting Defensively as a Standard Action gives a +2 untyped bonus to AC.
Per RAW, that does not apply to CMD because CMD gives an explicit list that does not include untyped bonuses to AC. It`s pretty clear that the RAI is that all bonuses that apply to Touch AC, including untyped bonuses, should apply to CMD, but the current wording doesn`t say that (Luck Bonuses were just added in the last Errata).
I`m marking the FAQ button on this...
The BEST fix would be to change the wording on CMD so it just says `all bonuses which apply to Touch AC apply to CMD´,
but Fighting Defensively as a Standard Action could also be Errata`d to be specified as a Dodge Bonus to AC.
That is kind of like the ``plugging the leak in the dam with your finger`` solution,
because there is just going to be an awful lot of untyped bonuses to AC out there,
and many of them are cases where they SHOULDN`T be typed as a Dodge bonus (like Fighting Defensively clearly should)

A creature can also add any circumstance, deflection, dodge, insight, luck, morale, profane, and sacred bonuses to AC to its CMD. Any penalties to a creature’s AC also apply to its CMD. A flat-footed creature does not add its Dexterity bonus to its CMD.
You can defend yourself as a standard action. You get a +4 dodge bonus to your AC for 1 round.
Dodge bonuses apply.
You can choose to fight defensively when attacking. If you do so, you take a –4 penalty on all attacks in a round to gain a +2 to AC for the same round.
This is untyped. Error? I would think so. Should probably be a Dodge bonus as well, but it could be argued as an Armor bonus, which would not apply.
