I'm starting an obituary thread since someone suggested one on another thread, i'll start with my own.
Zordius Jeggare, Tieling Warlock 3(male witch)
was slain by a gibbering mouther, at the temple of Zura, the fell down into the holw where the soul dolls were located, 5 minutes later the mouther came to investigate, one player's eidolon was destroyed before it turned towards Zordius who was trying to perform melee touch attacks on it, he was quicly bit, grabbed, and the one turn later engulfed, dying at -13 hit points because of his reduced con of 11, unfortunately afterwards his soul was consumed by the unholy altar at the end of the dungeon.

we didnt really do a dang thing with the "other castaways" on the shiv because we thought we might have to replace characters so they stayed mainly 'shadows' and didnt need to use them.
Until now.
Now we have a female elf witch (shadow Patron)
and a male half-orc barbarian/oracle (yep going for rage prophet) who was a slave, so he has chain fighter as a racial trait.
they are really cool characters.
elf might go mystic theurge (witch/oracle)
but more likely witch/shadow dancer.
cool things came out.
the guy who almost always plays archers decided to go rage prophet.
the girl who always plays combat types made a witch.
so we broke some molds. all in all pretty cool.

before we started playing again, well, adventuring, the party was able to raise the dwarf ranger from the dead. which was interesting because that player already made another character.
So he decided with the experience Levels he would have earned, he made a really cool adjustment:
Combining the character concepts:
The dwarf started life as a ranger, but was indeed captured in a cannibal ambush (resulting capture, and torture before being rescued, cause him to go a little nuts), so he took a level of barb.
His death in the battle that takes place in front of the whispering, bleeding, haunted, scary altar? Made him haunted and he took a level of oracle.
Really cool plot to come up with from the ranger. So hes now a ranger/barbarian/oracle (still eventually going for rage prophet) but a really cool development in the story.
One of the players quit, so, the other player decided to combine both characters into one as well (making a witch/druid) which is also really cool character.
Moving forward we happened upon an incorporeal foe in the jungle, which again ended in some deaths.
The witch/druid was able to be reincarnated and is now a half elf, rather than a dwarf.
The Alchemist was able to be reincarnated and is now human (yea the both got taller!)
So party has morphed!
we now have:
Dwarf Paladin
Dwarf Ranger/Barbarian/Oracle
Human Alchemist/Inquisitor (picked up a level in something else, alchemist alone was a might bit boring)
Half-Elf Witch/Druid (nature Patron, got switched from shadow)
Oddly enough these are still the original "minds" of all the characters that started out. The only actual "loss" was the Zen Archer.

we havent used reincarnate since 1st edition, was kinda fun that we found some scrolls in a magic shop and bought them.

Having followed the tracks of Ieana and Kovack clear across the island from their campsite, the party arrived at the lighthouse while still at Level 1. Dispatching the Shiv Dragon guarding the camp, they rounded the corner to find the village eerily deserted, though the Ranger could have sworn he had heard voices just minutes before.
Sensing a trap, the party heeded their dwarven instincts to hold the chokepoint at the eastern edge of the firepit area, sending the Ranger and Fighter to the fore with the cleric close behind. Hogan remained in the back, ready to step in for the timely sneak attack.
The cannibals, fed up with waiting for the prey to walk into their ambush, sent half their warriors to the fore while a handful of the braver (and hungrier) warriors crept along the roof of the northmost building, jumping down behind the unsuspecting Hogan and swarming him. Seconds later, his horrified brothers glimpse his severed head, the unmistakable trace of terror evermore etched into his features. They had barely time enough to register the loss before they themselves succumbed to their wounds...

you didnt give them a perception roll to notice a gaggle of barbarians comming their way?
we used the same choke point on the map.
difference is we were not level 1 when we got to the cannibals!
The monk went high right off the bat.
We took a few out with ranged attacks at the same time we fought that dragon.
we pretty much stayed at the choke point, used ranged attacks
dwarf pally had goood AC (21) so he tanked (they missed ALOT.)
druid kept using entagle it was hilarious.
Currently in this AP
My Dwarf Pally got sqaushed (took twice his remaining hit points left in one swipe) and damage went past his con by FAR.
so I made an elf fighter.
i rolled really good stats.
so hes awesome with a bow and two weapon fighting.
party rescued him from an encounter in the jungle where it made sense the monsters would have had a prisoner, i chose elf because he could have been a prisoner a long time, and id already done dwarf! lol
its kinda funny because we had a raise dead scroll, but the player who had it forgot about it.
later on...
the dwarf ranger got killed by a gorilla, which was kinda dumb. The witch was healing everyone, and basically forgot to heal the ranger from a previous fight, so she wandered into the next fight with somewhere around 16 hit points.
it was another one of those things where the round that brought her down also went way past her con score.
but on yea! they had a raise dead scroll when she went down (I see how it is)
so half way through part2 our party is:
Half-elf Witch/druid 3/3 (reincarnated from dwarf)
Elf fighter 5 (w00t Im so cool)
Human Inquisitor/Alchemist 4/2 (reincarnated from dwarf)
Dwarf ranger/barbarian/oracle 2/1/1 (lost two levels from raise dead)
so we are down to ONE dwarf (barely)
we are also really thinking of bringing back in a 5th PC.
I dont know what though. The inquistor/alchemist sort of takes the whole rogue slot up really good.
I can see something like a cleric/rogue, or sorceror/rogue ( i wish someone would play an arcane trickster, ive never seen one in live play)
someone babbled about a summoner or a cavalier, both odd choices...I Like odd!
could you see adding a cavalier to the party??

Circumstance: Killed in ambush by Faction agents hired by the Aspis Consortium.
The party consists of:
Toushiro (Human Paladin of Irori)
Erzbet (Female tiefling rogue)
Brenith (Elven ranger)
Ewan Starstrider (Human Cleric of Desna)
They have joined the Pathfinder faction.
The agents were hiding in the home of Cheiron the dwarf waiting for the pc's to arrive. As soon as they heard the pc's go up the stairs they used potions to turn invisible and positioned themselves for an ambush.
The agents had already rigged the doors to the pc's bedroom with glyphs of warding. When the paladin triggered one of the traps the agents suddenly attacked with poisoned shortswords. The elf got paralyzed by the poison while the paladin got hit with two sneak attacks. The cleric also got hit with one sneak attack. the next round the agents repositioned themselves and managed to surround the cleric for a double flanking. The poor cleric got slaughtered with three sneak attacks including one critical hit. The rest of the party managed to put down two of the agents before the remaining two surrendered, but the cleric was already dead.
They managed to persuade N'kechi a priest of Gozreh to use a scroll of raise dead they had found on Smuggler's shiv to bring the dead cleric back to life.

Causality: Ewan Starstrider, Human Cleric of Desna (again)
Circumstance: Killed in ambush by Mzali rangers.
After being hit with three human bane arrows in the surprise round, poor Ewan once again kicked the bucket. To get him back N'kechi took him to an old witch doctor living in a secluded hut in the screaming jungle. Using a strange JuJu ritual involving voodoo dolls and a blood donation from the party's elven ranger she reincarnated Ewan as a half-elf. The only drawback was that Ewan had to give her his firstborn child.

My first PC death resulted when the party's Elf Alchemist decided that it would be a good idea to approach the Treasure Pit on his own. They broke through the cork, and lowered him down on a rope, which he promptly stated that he released when he hit water.
Given the circumstances (6 players) I had increased the number of Ghouls to 3. He was second level at the time, went to 3 in the surprise round and lost the initiative roll for the first round and died. The second PC I lost, the party's Gnome Summoner, survived the encounter, but contracted Ghoul Fever.
Shortly thereafter, the PCs made their way back to camp to pickup a replacement (I had allowed a few members of the crew/extra NPCs to survive the initial wreck after reading how deadly this adventure could be), and headed back out.
The Gnome Summoner, sadly, made both saves for the Ghoul Fever. 4 game days later the PCs had made it to the area where sacrifices are left for the RMD. They decided to spend the night in a nearby (appropriately placed) cave (which I explained as a small cave that was covered in bones/gore from previous sacrifices after them making a religion check). A random encounter resulted in 3 shocker lizards.
28 points of damage downed the Gnome summoner, while the rest took 14. The next blast from the lizards dealt 16 points of damage, killing the summoner and dropping 4 members of the party to negatives. The only person on his feet was the barbarian (who had replaced the elf alchemist), who took down the remaining 2 shocker lizards.
He remained holed up for 3 days (during which I did roll random encounters, of which 5 triggered- talk about bad luck!) With only the party's fighter waking up on day 2.
They ofc returned to camp, picked up a replacement, discovered that Aerys (who in 2 weeks of play had not made a single morale check) was missing, and went gungho into the jungle after her.
Arriving at the Cannibal camp they faced 14 cannibals plus Klorak, and the barbarian (who had protected them all while they were unconcious) was left after he fell to negatives and the rest fled.
He was then dumped into a stew pot (essentially blanched) then dumped out into the dirt and had his chest cracked open so Klorak could eat his heart and rub the barbarian's life blood upon his chest, and now is suspended over Klorak's "throne" so that his blood may rain down upon the chieftan (what little is left) and off they trek back to camp!

Classes/levels: 4 Rogue
Adventure: Racing to Ruin
Location: Flencing House
Catalyst: Not knowing when to run
The Gory Details: Alred the rogue has had incredible luck so far. He's gone toe to toe with opponents that should have beaten him but up until this point has always managed to come out alive. The plan to rescue the hostages was most of the party to create a large diversion out front while Alred snuck in the back and freed the hostages. Again he got extremely luck and managed to kill both hostage takers in the room on the surpise round and first round of combat. His luck ended there though as the cleric leader retreated from the roof top and ran smack into him. Alred. The evil cleric flubbed a couple concentration checks but finally got a Hold Person off and held poor Aldred. He blew his save the next round as well and got his skull caved in by the clerics hammer. We managed to rescue 2 of 3 hostages but that was the end of our Rogue. He's now a barbarian keeping the party very melee centric. (Ranger, Cleric, Barbarian, Monk, Bard).

The party was searching the mercantile district when they saw a bunch of Shackles affiliated freebooters fleeing from three chimera's. The pc's immediately engaged the beasts who in turn answered by burning the entire party with their acidic breath weapons and killing most of the freebooters in the same go. The chimera's then entered into melee cornering the party next to a ruined building. The paladin was occupied with the biggest chimera while the other two went for the ranger and the cleric. After a few painful wounds the last chimera was still fighting the ranger and got in three critical hits in one round (using the critical hit deck). the first critical took off the elf's hand, the second crushed his foot and the last ripped off his ear, in the end the ranger fell dead from shock and blood loss after loosing his hand. the party held a memorable funeral scattering his ashes to rejoin the spirits of the jungle. The party has now met up with a sleazy pathfinder Magus named Vito to fill the shoes of their fallen comrade.

the party got separated in different rooms of a ziggurat (usually a recipe for disaster)
The alchemist/inquisitor and the rage prophet got jumped by keches (one fight)
The cavalier was investigating copious amounts of bat dung
and the magus and witch were poking around and got jumped by the three chimeras.
Big bad chimera breathed on the magus but couldnt catch the witch int he area of effect.
witch moved up to heal the magus and the other two chimera charged.
Both the witch and the magus critted on the AoOs on the same chimera.
all the while the inquisitor or the rage prophet are fighting keches.
long story short, by the time the cavalier had figured out there even was a battle, the magus and the witch had basically tooled all three chimera.
Cavalier got into the battle to be able to finish off one of them.
More or less two party members wrecked three chimera.
The witch and the magus are a nasty combination

It would probably have gone much smoother if I hadn't rolled a crapload of 20's. I use Cthulhu glow in the dark dice from Q-workshop as my DM dice and in this AP every fifth roll seems to be a nat 20. My players have threatened to steal my dice.

Name: Mattrum
Race/Class: Human fighter 2/ranger 2
Adventure: Souls for Smuggler's Shiv
Location: The Temple of Zura
Catalyst: Gib Gib Gib
Mattrum was the party's designated trap-springer, so he was the one to try to jump over the pit that the party's actions had opened. He failed to make the jump and plunged into the depths. He spent enough time scouting around and poking the weird narrow tunnels to stay within range of the gibbering mouther's confusing babble as it crawled to get him. His confusion was such that he could not grab the rope helpfully lowered to him until the beast was already upon him, and the aberration swiftly blinded him, making it difficult for him to grab the rope. The dwarf monk Losk was lowered in to retrieve him, but was himself blinded by the mouther's caustic spittle multiple times. In the end, a combination of multiple bites and a bomb thrown by the party alchemist Meg (she missed) brought Mattrum into unconsciousness, and the mouther embraced his prone body to feed. The party, pragmatic as always, let the mouther drink its fill and leave before descending into the pit and casting raise dead from the scroll found elsewhere on the island.

Catalyst: Itombu the shadow demon
After slaying the possessed dire ape, Marcus -- a collector of gruesome trophies -- strode in and began gutting the kill. As his blade carved through the dead beast's flesh, something odd happened. A black oily mist began to ooze forth from the ape's open mouth, it's notrils, and the dozen or so wounds Marcus and his companions had dealt it. A moment later a shadow demon stood before them and a desparate melee ensued. Though the party defeated the demon and sent it howling back to the Abyss, Marcus was not so lucky. A shadow-spawned fireball escorted the brash rogue immediately into the afterlife. His final resting place was a hastily dug grave along the muddy riverbank.
GM's note: This was the first death of a party member for this campaign. I think I'm improving. ;-)

A 4th level character, probably Dex based and the designated trap-springer can't make a DC 15 Acrobatics check?! He had it coming...

The camp was attacked by two rampaging camalutz as a result of a random encounter. Ewan moved into range and cast a spiritual ally spell to strike at the beasts. The parrot fiends got the drop on him with their great flying speed, flanked him and one dealt a critical hit giving him massive bleed damage. The next round one of the camalutz finished him of with a flurry of beak and claw attacks. He later got reincarnated (for the second time) by Osond and came back as an elf.

Name: Bahir the Savage
Race/Class: Half-orc Druid 2 / Rogue 4
Adventure: Racing to Ruin
Location: Tazion
Catalyst: Raogru
After watching his minions fall before the PCs, Raogru saw the writing on the wall. To buy a little time, the charau-ka priest removed one of the wooden planks that allowed passage across a noisome tar pit and effectively put a 15-ft wide moat between himself and his enemies. Undeterred by the obstacle, Bahir and his faithful raptor companion Azziz each made a running leap and successfully landed next to a now very disappointed Raogru.
Unfortunately, none of Bahir's companions had the ability to swiftly follow him. Bahir struggled against Raogru's poisonous stares and devastating heavy mace strikes. Then, Raogru successfully cast dominate animal on Azziz and turned the savage little beast on his now flanked master. Within 3 rounds, Bahir lay unconcious and bleeding at Raogru's feet. A smirking Raogru then performed a coup d'grace on the helpless half-orc, and ushered him swiftly into the next life.

An old chaura-ka cleric brought a huge statue of Angazhan to life and had it fly down to assault the pc's as they were in the midst of a furious battle to overthrow the chaura-ka and take over the military district. With them was about fifty sargavan soldiers and a dozen pathfinders including General Havilar of the Sargavan government and Jask a cleric of Nethys rescued by the pc's on Smuggler's shiv. The demon ape landed with the chaura-ka perching on his soldier. The paladin quickly moved in to smite the foul beast. On the second round of combat the demon managed to get a critical hit on the paladin and using the critical hit deck I got the head severed result. Against all odds the paladin failed his saving throw against the decapitation effect, but luckily Ewan,. the party's cleric of Desna had a br5eath of life spell prepared and managed to put the paladins head back on and bring him back into the action.

Name: Sujiu
Race/Class: half-elf fighter (weapon master) 4
Adventure: Racing to Ruin
Location: The M'neri Plains
Catalyst: A night attack by a pair of chemosits.
The party passed by the village and ended up being ambushed by the chemosits during the night. One of them went for the guy on watch, the other for the sleeping Sujiu. It was over pretty quickly.
Sujiu was then reincarnated into the body of a gnome in Kalabuto. Since this made him unable to use his heirloom weapon, a Medium-size composite longbow, he set out on his own to find someone capable of casting wish and returning him to his former state.

Race/Class: Half-orc Druid 2 / Rogue 4
Adventure: Racing to Ruin
Location: Tazion
Catalyst: RaogruAfter watching his minions fall before the PCs, Raogru saw the writing on the wall. To buy a little time, the charau-ka priest removed one of the wooden planks that allowed passage across a noisome tar pit and effectively put a 15-ft wide moat between himself and his enemies. Undeterred by the obstacle, Bahir and his faithful raptor companion Azziz each made a running leap and successfully landed next to a now very disappointed Raogru.
Unfortunately, none of Bahir's companions had the ability to swiftly follow him. Bahir struggled against Raogru's poisonous stares and devastating heavy mace strikes. Then, Raogru successfully cast dominate animal on Azziz and turned the savage little beast on his now flanked master. Within 3 rounds, Bahir lay unconcious and bleeding at Raogru's feet. A smirking Raogru then performed a coup d'grace on the helpless half-orc, and ushered him swiftly into the next life.
This seems like an unlikely happening, Raogru is flanked by a 4th level rogue and an animal companion, he should be in serious trouble.
The other party members can't jump across but they should still have ranged weapons and spells, no?Venomous stare doesn't provoke AoO? The spell casting would. I don't see how Raogru's action economy would have won out here, he should have gotten tooled.... what happened??

This seems like an unlikely happening, Raogru is flanked by a 4th level rogue and an animal companion, he should be in serious trouble.
The other party members can't jump across but they should still have ranged weapons and spells, no?Venomous stare doesn't provoke AoO? The spell casting would. I don't see how Raogru's action economy would have won out here, he should have gotten tooled.... what happened??
I'll try to be brief...
Early on, Raogru had cast cause blindness on the party's archer (elf bard/ranger) which took her out in Round 2 of the combat. The rest of the group (dwarf fighter, dwarf wizard, dwarf cleric, and Pezock [fighter/rogue]) were engaged for the first few rounds of the Bahir-Raogru fight finishing off the last of the charau-ka minions. The cleric and Pezock both attempted the jump and ended up stuck in the tar pit. The fighter (the cleric's cousin and protector) abandoned the fight for about 3 rounds to rescue them. The wizard (an earth elementalist) had no way to cross the tar. He tried a create pit spell, but Raogru avoided it.
Azziz rolled pitifully against Raogru (who has a decently high AC). Bahir hit him a few times, but Raogru had more HPs. Plus, Raogru was able to take several 5-ft steps to safely release his spells and venemous stare ability. He was never successfully flanked by Bahir and Azziz either. When Azziz got dominated, that was all she wrote.
The fighter eventually climbed along the ruined wall above the tar pit to reach (and slay) Raogru, but he was too late to save Bahir.

The ninja turned invisible and split off to the right to flank with the rogue. The rogue followed suit and went left. The rest of the party prepared for the fight by readying attacks and casting spells. The next round the T-Rex came into full view, the rogue unfortunately had moved just far enough forward to be in biting distance and got the not quite as coveted title of "only thing in range". 4d6+22 damage later the rogue was at 15 HP and in it's mouth. Next round the rogue was swallowed and knocked to -17, killing him.
For what it is worth the party used the round the T-rex spent swallowing the rogue to knock the T-rex to -1. My group's damage is getting out of control, the samurai alone is dishing out ~24 damage a swing (and is normally hasted).

a group of 5th levels took the t-rex out in essentially one round of melee attacks? That's a Cr10 critter. Do you have more than 4 in your group?

It's only a CR9... like I said my party has some high damage potential. At the time of the fight the party was made up of a ninja, samurai (samurai 4/fighter 1), rogue, paladin, wizard. The wizard drops haste every fight, upping the damage of the party pretty drastically. The samurai does 1d10+19 damage to his challenge target, the paladin deals 1d10+13 damage, the ninja is sneak attacking just about every turn and dealing 4d6+3. The paladin scored a critical on her attack, so it took 3d10 + 4d6 + 48, the paladin and the samurai both rolled high on all their damage dice (8,8,10) This is about 90 damage already, and I did forget to mention that the samurai and paladin had shot it with arrows, not readied attacks (hasted they dealt a combined 4d8+20 damage, maybe more i forget if the samurai had challenged at that point.). The wizard summoned a monster that dealt out some damage as well. Overall it comes from a high damage potential and lucky rolls for both attacks and damage. It should be fixed from now on since they just reached Tazion and no longer only have 1~2 encounters a day I can actually bleed their resources so they aren't hasted every damn fight.
