This is a fairly basic question, and it probably has a simple answer, but while I was looking through the core rule book, I noticed that gnomes get a +4 dodge bonus to AC against monsters of the giant type (this is done via Defensive Training). Does this bonus also apply to creatures that are larger than giant (massive, gargantuan, etc.)?

Not Giant size, but typed as giants.

(Giant): cloud giant, cyclops, ettin, frost giant, fire giant, hill giant, ogre, ogre mage, stone giant, storm giant, troll

Giant used to be a type. It is now a subtype of humanoid. The gnome entry on this should be seen as needing errata (if it isn't already done). The bonus applies to all creatures with subtype (giant), regardless of their size.
As a side note, the GM in me smiles in evil-mastermind-glee at the fact that giants, ogres, etc. are now a valid target of enlarge person.

And reduce person. Last thing you want to see as a player from the bandits that ambush you is for them to get bigger, twice!
