If everyone fails their save, everyone in the room is screwed.

Everyone makes a save on their turn. Only if the creature spends a standard action to target a single creature with his gaze attack does that target need to make a 2nd save against the gaze (on the gazer's turn).

Using the total defense action is a popular tactic for intelligent creatures with gaze attacks.
