Can you use the drag combat maneuver (APG 321) offensively like you can bull rush? Unlike the reposition combat maneuver (APG 322), it doesn't seem to have any limitations about positioning the opponent into or through hazards.

For example, can a fire giant drag an opponent into a nearby river of lava?

Could a dragon swoop down with flyby attack and drag a smaller opponent high into the air to be dropped?

Could a Yrthak or similar flying monster drag a land bound opponent off a nearby cliff?

Could a fighter with electricity resistance drag an enemy into his wizard friend's lingering lightning bolt?

Why or why not?

The way I read it the answer would be yes to all of these. Although I might give the person being dragged a grapple check to try and hang on.

If a person was dragged into the air by a dragon, can the dragon continue dragging him round after round for additional height? What happens if he fails the check? The target falls? Is it even possible to drag someone off the ground, and if so, does it last longer than the action it takes to do it (effectively dropping him from a few dozen feet at best).

Grappling is probably much more proficient for the grab and drop maneuver anyways.

I would do it as if they fail the drag check the character can make a grapple check to hang on. Another idea would be with flyby attack if the dragon grapples he gets to make a free drag attempt. That way the grapple wuld be on going well he is dragged/
