It will change from month to month. The AP and Chronicles are monthly, while the Companion and Adventures are bi monthly. In theory the last two switch off. But sometimes they are in the same month, sometimes they will do one two months in a row and miss a month another time ect.
On Avg you will pay around 55 a month with shipping. ((shipping in the US that is, over sea's is higher)) But some months will be less and some months much higher from time to time.

In general its pretty standard, but that's assuming everything comes out on schedule. The next few months, for example, will be much more expensive than the standard month because so many products (from as far back as June) are being released late due to the extra time and effort needed to get the Core Rulebook and Bestiary launched over the summer. You can see what your upcoming months' anticipated costs will be in your My Subscriptions page, though you have to have the subs already for them to show up here.

As Dark Mistress and yoda have said... yes, it will vary from month to month depending on release dates (which can vary in and of themselves). Your upcoming subscriptions will be mapped out for you (actively updated using our best, most up-to-date knowledge) on your My Subscriptions page.
The only thing I have to add is that you do not need to have any subscriptions to take advantage of this. The same information that informs your My Subscriptions page, also informs our Product Schedule. So you can see all of our upcoming products for the next several months.

Say I want to subscribe to Companion, and each time the subscription comes around I want to maybe add one or two items--a mapset or a couple of minis or something.
Does the process allow for all of these to ship together to save on shipping?

Yes if you order something, say from Green Ronin you can have it ship with your next Paizo subscription order. I do that all the time. I recently bought a bunch of dungeon and dragon mags I didn't have over the last 3 months and would have them set to ship with the next subscription order to save me on shipping and handling.
You just have to make sure you order the other stuff and set it to ship with the next subscription before the current on is being processed. I had that happen once. I waited to long so the extra stuff went out the following month.
