Just making sure I have the math right. Let's say a fighter with a BAB of +11/+6/+1 has TWF, Imp TWF, Greater TWF, and TWF Rend.

With BAB of +11, while using a light weapon in his off-hand, which of the following would be correct?

1:
+9 primary (-2 TWF)
+4 primary (-2 TWF)
-1 primary (-2 TWF)
+9 off-hand (-2 TWF)
+4 off-hand (-2 TWF, -5 Imp TWF)
-6 off-hand (-2 TWF, -5 Imp TWF, -10 Greater TWF)

2:
+9 primary (-2 TWF)
+4 primary (-2 TWF)
-1 primary (-2 TWF)
+9 off-hand (-2 TWF)
+6 off-hand (-5 Imp TWF)
+1 off-hand (-10 Greater TWF)

3:
+9 primary (-2 TWF)
+6 primary (no penalty)
+1 primary (no penalty)
+9 off-hand (-2 TWF)
+4 off-hand (-2 TWF, -5 Imp TWF)
-6 off-hand (-2 TWF, -5 Imp TWF, -10 Greater TWF)

4:
+9 primary (-2 TWF)
+6 primary (no penalty)

Or are not of these correct?

Also, for the purposes of TWF Rend, must a person make alternating attacks, with rend only coming into effect when a primary and off-hand weapon hit consecutively? Or can a person rend in any full attack that a primary and off-hand weapon hit, regardless of whether the hits were consecutive?

The correct answer is +9/+4/-1 (main hand) and +9/+4/-1 (off hand).

Two-Weapon Rend triggers in any round where you hit at least once with your main hand and at least once with your off hand. The order doesn't matter.

If this is correct, why does the -2 stack with the second and third off-hand shots, but the -5 does not stack with the third?

Look at it this way.

Your base attack bonus is +11. You get multiple attacks at +6 and +1, so your overall bonuses are:

+11/+6/+1

Two Weapon Fighting gives you an attack with an off-hand weapon, and reduces the penalties so that all your attacks with both primary and secondary weapons are -2/-2 (assuming light in off-hand). Your to hit now looks like this.

Primary: +9/+4/-1
Off Hand: +9

Improved Two Weapon Fighting gives you a second attack with a -5 penalty.

Primary: +9/+4/-1
Off-Hand: +9/+4

Greater Two Weapon Fighting gives you a third attack with a -10 penalty.

Primary: +9/+4/-1
Off-Hand: +9/+4/-1
