I have been working on converting the Rise of the Runelord AP into a CoC themed, modern adventure (set in future Kalamar, my setting of choice).
The Runelords are perfect as Elder Ones, buried deep in the earth and scattered about the globe. Skinsaw Men make a perfect CoC cult.
Throw in modern setting, equipment, weapons and TADA!

I've been thinking of CotCT done in a modern setting on Earth using MCWoD. It would need to be in a country with a monarchy or would require a serious re-write of the political situation. I would need to increase the power level of the encounters to accomodate the +3 level of MCWoD characters. Or I suppose I could just work all of them as a +3 LA and no one advances till fifth level.

How about a modern slant on the Seed of Sehan? It would need a little bit of modification but a drug problem in a city is certainly a normal modern day type of plot. However this drug has a supernatural connection and leads to two different cults, both of which are trying to use the drug and the supernatural substance it comes from to establish power and control.

I am currently thinking of converting CotCT into a d20 Future setting. No aliens, magic translates into cybernetics/nanotech, monsters are mutants and bio-engineered creatures.
Varisia is a frontier solar system, the Shoanti are disgraced remnants of a former colonial drive funded by a Megacorporation that is no more, Varisians are just the same, only with interplanetary travel.
The city of Korvosa becomes Space Station Corvus-A, Cheliax is a Megacorp that has undergone some serious managing troubles, and
Right now (as for Seven Days to the Grave), the project seems highly feasible, as many elements are easily translated into a space opera/sci-fi environment, but I'll have to see if future issues and adventures are more problematic or not.

That all sounds great. Terrific ideas. One question. Why no aliens? There are plenty of non-humans that could be aliens, but I suppose mutations are good too. Undead as mutants is pretty cool. Hellknights could be cyborgs.

I thought about including aliens (both for monsters and playable races) but I feared that the Star Wars syndrome was just behind the corner, along with the temptation of incorporating some fantasy elements.
Moreover I aimed for a darker-themed setting, with Megacorps like the Weyland-Yutani from the Alien movies and overall untrustworthyness like the one from the Neuromancer novel.
Plus, as a personal maniacal touch, there will be powered suits and mechas and starships.
Likewise, although there's nanotechnology, bio-engineering and a very developed and dangerous cyberspace, I don't want to include some more extreme elements such as FTL travel o even a hint of psionics.
Mostly, it can be summarized into the "say no to catch-it-all settings" concept. It seems to me more plausible if something is missing, rather than if there's a touch of everything.
