Is the only Monster Manual that is open content the 1st one?
None of the material in the Monster Manual is open game content. The monsters that are open game content that also appear in the Monster Manual are located in the Monsters system reference document on WoTC's site or a dozen other online.
Very few of WoTC book have any open game content in them.
Exceptions are:
Monster Manual II has two: Razor Boar (p.220) and Scorpionfolk (p.221)
Unearthed Arcana has a bunch of open game content if you read the declaration carefully.
Weapons Locker (d20 Modern) also has a good chunk of open game content as well (though its more restricted).
Otherwise WOTC open game content is found in the system reference documents.

MM II's OGL critters come from the Creature Collection, actually. They aren't Wizard's open content.
I think it's an evolution in policy we saw.
First stage, OGL, goodies for everyone!
Second stage, We'll show off other people's good OGL stuff (MM II, Unearthed Arcana)
Third Stage, Wow, we'd better stop doing that.
4th stage (St4ge?) Ok, pulling all these license home so we can do it all ourselves. and stop pointing out all the other cool stuff others have made.

This is why i really dont know how good the 4.0 OGL will be and i think Paizo should stick with 3.5/3.75
