I have never had to deal with a disease in D&D, and I don't have access to my books (well files) at the moment, so can someone tell me how Vorel's Phage works? Does the 1 day incubation mean that it takes one day for the disease to show signs or does it mean that the disease lasts for one day? If the incubation period means it takes one day to show signs, how long does this disease, or any disease last?

Initial Fort save is to shrug the disease off entirely. If that fails, after the incubation period passes, the initial damaging effects are done. Every day after, further Fort saves are necessary - two successful consecutive saves IIRC are required to throw off a disease in most cases. Which makes the diseases that affect CON especially nasty if you don't resist the initial ability damage... it becomes progressively more difficult to resist the disease on your own.
Heal checks for long-term care can be substituted for a poor Fort save character's save bonus, again IIRC. My players have used this to good effect sadly, staving off Ash Doom, huecuva blight and savage fever in multiple characters in thier first five levels alone...
edit: Locke pointed out something at least indirectly: diseases, generally, 'end' when they are kicked either by successful saves (or equivelant), being removed or when the ability score(s) they trash are zeroed. Certain diseases - notably lycanthropy and mummy rot from the MM, not to mention other supernatural diseases - tend to have really nasty effects if they are not dealt with very quickly...

Incubation is how long it takes to show signs. As for how long it lasts I don't have my books handy either but in my games it seldom matters remove desease tends to clear up the problem before too long.
