Map Pack: Dungeon Chambers Error

I got the map pack in the other day, but one of the map cards is printed twice, and one of the maps (one of the lava pool maps) is missing entirely. Anybody else have this problem? And how do I get this rectified?

I apologize for the defective item, I'll get a replacement sent out right away.

On a side note, this kind of thing is probably better suited to the Customer Service thread. If only for the reason that I'm more likely to catch it there. : )
