Harliss Javell isn't a leader in the Crimson Fleet. She's the captain of a ship that works for the fleet. The PCs don't actually meet any real Crimson Fleet pirates until Adventure #5, to tell the truth.
As for them having red eyes... while the majority of the Crimson Fleet pirates are human (if vile and reprehensible humans)... those rumors of red eyes have to have started somewhere, yeah?

Do ships that "work for" the Crimson Fleet fly the horned skull flag, or just ships with fiendish captains? (One of the characters, who was briefly a pirate, may well recognize it if it's flying on Javell's ship before the ship crashes into the walkway.)

The majority of the Criimson Fleet pirates are merely (really reprehensible) humans or humanoids. They're based in Scuttlecove, and while they have a sizable fleet, the majority of the ships on the seas flying their flags are mercenaries who work for them. Harliss is one of those mercenaries.
