im gonna put cauldron in the realms because the group im gonna dm for likes Faerun best.
Toril uses a different cozmoligy than greyhawk, not the great wheel from the DMG, so where should i put Carceri in the realms cozmol?
If I am not mistaken the forgotten realsm also uses the great wheel, so your set to go.

Toril uses a tree-like arrangement of the planes, unlike the standard D&D cosmology. There is no Carceri, or equivalence, in Toril cosmology, so you will either have to insert Carceri into the current cosmology, or substitute it for some other plane. The demodands in Toril comology inhabit the Supreme Throne, the plane of Cyric, so that's one possible alternative. However, I personally don't like that alternative, and do instead use the Barrens of Doom and Despair instead of Carceri.

You could also make Carceri a demi-plane with finite dimensions (after all it is a "prison plane").
Archidamus (sp?) and a big bunch of Demodands got caught and thrown into that plane, as well as punished spirits/petitioners.
That's not a bad idea. You could, for example, make it a demi-plane within the Barrens of Doom and Despair, originally created by Hoar.

I don't know the new FR cosmology well enough to comment, but having Hoar involved seems relevant, flavour-wise.
I guess that it depends where the DM sets the game, but one could even replace the church of St. Cuthbert with Hoar! It seems a near perfect match.

It is a near perfect match! In my Wizard's Reach campaign, I've replaced the Church of St. Cuthbert with Assuran (the Untheric name for Hoar). The magical mace with divination abilities is now that "Talon of Vengeance".
As well, I've replaced Wee Jas with Kelemvor (and some nasty Jergalites inside), Pelor with Lathander, and Kord with Ramathant (the Untheric name for Anhur). I've thrown in shrines to Ishtar and Kossuth as well.
