I'd probably err on the side of letting it last at least 1 round. It's similar to Antagonize, which also doesn't addressing knocking the source out, but does say the effect lasts until the target can't see the source for 1 whole round.

I think the held in one hand "attached to a simple pole" overregulates it. It just needs to be held in one hand and can be attached to a weapon or shield. I understand this isn't what it currently says, but I think they should simplify the feature. I can't see where waving your banner around in your hand or on a pole is going to make a functional difference--your hand is occupied either way--nor whether your banner is affixed to a greatsword vs. a shield (or is your heraldry on your shield).

I agree with your interpretation. Looking at the playtest document, there is some circumstantial evidence that this is a result of sloppy editing. Many of the free actions say, "an immediate free action" but not Pincer Attack. However there are other free actions without "immediate" that don't necessarily make sense without the word "immediate" (Ready, Aim, Fire! Valkyrie's Charge is all kinds of poorly worded). And then there's the consideration that the Commander's signal is a technical trigger. That's not to argue your ruling but to point out feedback we should all include in our surveys.

As a GM, I would outline how long I was willing to wait at the front end, much like how GMs outline their general posting frequency expectations. As a player not playing a Commander, I find it easier to type out a quick post in response to the reaction or immediate free action than when it's time for me to post my full turn and want to think about my actions or how I want to word my post. :)
