"Precision-Based Damage (like Sneak Attack)

The following creature types (or subtypes) do not take additional damage from precision-based attacks (such as sneak attack):

Elemental (subtype): "<An elemental...> does not take additional damage from precision-based attacks (such as sneak attack.)"
Incorporeal (subtype): "An incorporeal creature is immune to precision-based damage (such as sneak attack damage) unless the attacks are made using a weapon with the ghost touch special weapon quality."
Ooze (Type): "<An ooze is...> does not take additional damage from precision-based attacks (such as sneak attack.)"
Protean (subtype): (50% chance to ignore, see below*)"

They aren't listed. They can take precision damage. The rogue will get lucky to get one in the whole fight, and even then only with tiny swarms, which still halve the damage. I could see letting them get one in.

Ohhh. Maybe because the party is snooping around looking for clues at the locations of the murders, they get pegged as the murderers. A hint dropped by the vampire lord, once he is aware of their involvement. From that point he could just mess with them with dark visions and make them start to believe that they are the actual killers.

Music is helpful. I like to keep a fresh 120 gigs of gaming music on hand so I have music for all situations. Desert/Combat/Big Cities/Scary Dungeons/Temples, etc.

So this issue came up for a halfling in one of my games when we found a medium creature sized light shield, so I made a pretty basic set of house rules to cover the gaps for the RULES to use a shield that isn't your size. It's pretty logical (in my opinion) and follows the basic guides of how Pathfinder tends to build their rules.

Characters are not proficient with shields that are a larger size category than they are. If they equip one, they suffer the penalties (to attack and movement based skills for lack of proficiency.

Using a shield that is made for a smaller character reduces all the bonuses and check penalties by half (rounded down)(though it must be adjusted to fit you.)

Using a shield that is made for a creature larger doubles the bonuses and penalties (rounded down). Again this is with the minuses to hit and movement skills for lack of proficiency.

To map that out:

A human using a light steel shield-
(Medium size) Shield, light steel AC +1, Penalty -1, 6lbs
(Large size) Shield, light steel AC +2, Penalty -2, 12lbs, -2 Attack, -2 movement based skills
(Small size) Shield, light steel AC 0, Penalty 0, 3lbs

A halfling using a heavy steel shield-
(Medium size) Shield, heavy steel AC +4, Penalty -4, 15lbs, -4 attack, -4 movement based skills
(Small size) Shield, heavy steel AC +2, Penalty -2, 7lbs

Well, there are many types of "madness" or "insanity" but in most stat based themes those are based on Wisdom. Less wisdom means less sanity. Low int means you are just... dumb. But you can play it however you need. I just think to really nail the insanity thing another non wisdom based class would be stronger. Low wisdom means you just don't notice things (Perception) and you don't pick up on things (Sense Motive). That aside, pretending you are playing insane can be fun!

I think some cool domain powers to use to illustrate your madness is to SPREAD your madness to your enemies. Eff with their heads. Two cool ideas for domain powers would be Luck: Curse. Drop -2 to saves within 30ft. They don't get a save. Then use Command on them to make them do whatever you want. I love using Command to make them drop prone, especially when they are surrounded by fellow party members. And since it sounds like most of your party is close combat characters, that would be great for giving them a round of +4 to hit, then extra attacks of opportunity when they stand back up.

Another cool one is Trickery: Deception. When they miss you in close combat you can shift around them (like a mini teleport). If I swung at someone and they vanished, I would think I am losing my mind. Also this helps to position yourself to give your allies flanking bonuses to hit.

Just a few ideas. I say when you find cool things, braid them into your beard. Jingle jangle.

thought that doesn't include the rules for 2HF. That is without them. Increase each by 7 with that ruled included. So 22-32

Nothing wrong with having them fight another group of level 18-20's. An evil group. Yes it is a lot of work to run a group of evil characters if it is 4 character sheets to be juggling each combat round, but if your PC's are tired of the same old Dragons and Mind Flayers, maybe let them face opponents that are truly on the same playing field as they are.

If It works with the story, having a big badass character on a big badass mount is a nice end game too.

Or if you have the table space, maybe have them take part in a battle between the heavens and hells? Lots of angels and demons on a huge battle mat, with the party stuck in the middle. Oh no!
