My experience has been that if you Wish for a roast beef sandwich, it's probably going to destroy the world. I would never voluntarily use wishes, even for the things listed in the rules.

The general way things are handled is that whatever interpretation would hinder player characters the most is the correct one.

I suppose it would be possible to destroy the Tarrasque by using a Wish to negate all strong nuclear interactions. The result of this would be that all matter would effectively fall apart releasing the binding energy as radiation. Even if it reformed after three rounds, the lack of the strong nuclear force would cause it to detonate in a massive nuclear explosion once again. This could only happen with a player sufficiently disengaged from the campaign to make such a wish, and a GM sufficiently disengaged to allow it to be granted. Total apathy is one thing that not even the Tarrasque is safe from.

You could always use the Stone Call and listen for the outburst of profanity when it hits the invisible target. This would at least give a clue.

In a 3.0 game with rolled stats instead of point buy, I put my rolled stat of 8 into Constitution for a Sorceror. I explained that a magical accident had triggered his inborn powers but damaged his health in the process. He selected a toad familiar to use the strength of the magical bond to offset that weakness, but I never described him as being particularly robust. I haven't done anything like that with a point buy yet, since I'm one of those people with a strong aversion to stats below 10.
